Partnership for Health Management


"I developed a passion for this organization because I love working with people," said Claudette Johnson, President of Partnership for Health Management.    "I love to help people and provide services where they're needed."  

In a nutshell, that is exactly what Partnership for Health Management does.  The organization increases access to healthcare through disease management, case management, and population management services.  

In creating accessibility Partnership for Health Management empowers patients to be more proactive in their healthcare and educates them on how to navigate community resources available in the healthcare system. "With the economy as bad as it is, more and more people are uninsured or going on Medicaid," said Johnson.  "We fill that gap in the community."

The current structure of the healthcare system in the United States is a serious concern for many people.  There are rigid restrictions in terms of individuals with chronic illness or preexisting conditions.  Individuals with these health problems also have a difficult time securing affordable insurance.  "The system is in need of a complete overhaul," said Johnson. "I'm glad to be apart of the bigger picture in what healthcare needs to be."
